Checklist for filing -  bring these papers with you!
 

Please bring all of the requested information regarding your income. Although all of the income may not be subject to income tax, it is necessary to determine if you can receive free income tax assistance. Also, do not forget your husband’s! wife’s income if you are filing a tax return together.
BOTH HUSBAND AND WIFE MUST BE PRESENT TO SIGN FORMS IF FILING ELECTRONICALLY.

 

  • Picture identification: Driver’s license, state ID, student ID, etc.

  • Social security cards and birth certificates (or other proof of birth dates) for everyone

  • Wages: Form(s) W-2 from your job(s)

  • Interest from bank and/or credit union account(s)

  • If you received less than $10 of interest from this/these account(s), please look at the bottom of your December 2004 statement.

  • Dividends from stocks

  • If you received less than $10 of interest from this/these stock(s), please bring the stub(s) from the check(s). If you did not receive a check please bring all statements from the company with which you have stock.

  • Unemployment: Form 1099-G

  • Bring in Form 1099-G that shows how much unemployment you received. Also bring any information if you had to repay unemployment.

  • Pension: Form W-2P and/or Form 1 099-R

  • Supplemental Security Income (SSI): Please bring any letters and statements for SSI received during the year.

  • Social Security Benefits/Retirement/Survivor Disability (SSDI): Please bring Form SSA

  • 1099, which is issued by the Social Security Administration. If you had to repay any money, please bring proof.

  • Family Independence Agency: Please bring in any letters and benefit statements for the year.

  • Friend of the Court (Child Support): Please bring any documents that show how much child support you were paid.

  • Gambling Winnings: Form W-2G

  • This is the document issued when you have lottery or casino winnings.

  • Self-Employment Income:

  • Please bring in any and all records showing income you made from cutting lawn, selling products, cutting hair, shining shoes, providing nail service, etc.

  • Other Income: Please bring in check stubs for income such as jury duty pay.

  • Alimony: Bring in all information about the alimony that you received. (If you paid alimony bring in the name of the person you paid, his/her social security number and your divorce decree.)

  • Property tax receipts, rental receipts and the name(s) and address(es) of landlords/landladies.

  • Proof of amount paid for heating bills from November 2003 until October 2004.

  • Receipts for money paid for childcare. Please provide identifying number of the person or business that provided the childcare, as well as the address.

  • Receipts for tuition payments and/or student loan interest. If you received a scholarship, please provide the information.

  • Account and Banking routing numbers for direct deposit of your refund.
